I can not get the medical history of my husband died in Los Angeles. Who I can contact? Help!?
He died at LSU Medical Center in Shreveport in late December. I was out of town and was taken there after a car accident and died 10 hours. later. I know that surgery was performed, and died shortly afterwards. I made several attempts to complete forms (even drove there from Houston, Tx to sign in person. At this time I am asked again with a death certificate, certified marriage license our son was born, social sercurity documents for the surviving spouse. Who I can contact for help in this matter. Certainly, there are laws that give me the right to information. Thanks in advance for any information.
Are you the personal representative or executor of your estate husband? I guess I knew of the properties unless you have a trust. If you have served as a public relations you should have something like the letters Will or other legal identification documents of names public relations assets. This document is basically his "authority" to gather information and act on behalf of the estate. If you are not the PR person is probably the best person to ask for documents (as the rear it may seem). The court document, with the death certificate or an authorization to disclose information, you must have records that need. Sure that the direction of the department of medical records.
